To upload your evaluation, click “Take Quiz” and it will guide you through a step by step process to upload all necessary photos and documents for review and feedback. If you have any issues with uploading, please contact us through your Student Notes widget for assistance.

Each question in the quiz will allow for 1 file upload, and will have a description of what is required to move onto the next question. Once you have uploaded all files, an instructor will review your submission and provide you with feedback and a final grade for the submission.

All evaluations must be in PDF format, while images can be PNG or JPG/JPEG.
